The 4 requirements of the COI are shown below:
1. Policy Effective Date – must be in effect on the date(s) of the event.
2. Minimum Insurance Coverage Required - $1,000,000 combined general liability coverage for bodily injury and property. *$1,000,000 products liability when food is sold or prepared and served. **$1,000,000 automobile liability only for food trucks or any vendor using a vehicle or trailer during an event.
3. Certificate Holder – must show as Boyertown Area School District, 911 Montgomery Ave, Boyertown, PA, 19512.
4. Additional Insured – COI must indicate that Boyertown Area School District is also listed as an additional insured under General Liability and (if applicable) Automobile Liability.
